Speech Language Pathologist (SLP)
Flagstar Rehab is looking for a Speech Language Pathologist to work in a skilled nursing facility in White Plains, NY. This position offers flexible scheduling and the option for independent work, with remote onboarding and quick placement.
Compensation & Schedule Hourly Pay Rate: $46 (W-2), $57 (1099). Flexible schedules (Full‑Time, Part‑Time, Per Diem, Weekends, Coverage, Days, Evenings). Pay is issued weekly via direct deposit.
Responsibilities
Provide efficient speech or swallow evaluations and/or screens of patients.
Develop plan of care, downgrade/upgrade as appropriate, manage meal texture.
Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
Monitor and document patient progress using innovative EMR software.

Not accepting Clinical Fellows (CFY) at this time.
Current NYS SLP License and Registration.
Clinical Competence Certification (CCC).
CPR Certificate.
EMR Competence.
Meets all State and Federal requirements for US employment in a Nursing Facility.
Malpractice/Liability Insurance (if applicable).
