Speech Language Pathologist (SLP) in Skilled Nursing Facility

Flagstar Rehab is currently staffing full‑time Speech Language Pathologists for our Rehab Department in state‑of‑the‑art skilled nursing facilities throughout NYC and surrounding areas. Flagstar Rehab SLPs work with a diverse, multi‑disciplinary team to perform evaluations, treatments, and patient/family education, improving speech, swallowing capabilities, and overall functional status. We are NOT accepting Clinical Fellows (CFY) at this time. Employment options: Direct Employment with our agency or independent contractor. We can assist you with setting up a corporation free‑of‑charge. Ask us how! Hourly Pay Rate: $46/h (W‑2), $57/h (1099). Responsibilities
Provide efficient speech or swallow evaluations and/or screens of patients Develop plan of care, downgrade/upgrade as appropriate, manage meal texture Provide quality therapy services to individuals in an SNF setting, caring for patients with TBI, CVA, stroke, Parkinson’s, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses Monitor and document patient progress with the use of innovative EMR software. Proficiency in Rehab Optima + Point Click is a plus. Benefits
