— Part-time; Every other Saturday and Sunday 8:00am - 2:00pm. Responsibilities
Assess and identify patient/caregiver problems, establish nursing patient/caregiver care goals, and identify and implement interventions for all agency programs. Document outcomes of care. Work with physician to establish and implement medical plan of care for the patient. Collaborate with interdisciplinary team; communicate effectively with patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations. Establish and maintain therapeutic relationship with patient/caregiver to provide emotional support and teaching. Qualifications
Education and Experience: Graduate of an accredited School of Nursing Must be currently licensed as an RN in North Carolina and in good standing with NCBON. One year of experience in an acute, hospice, home health setting, or long-term care setting. Other: Valid state-issued driver’s license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation. Summary
