PRN RN/LPN Home Care - Hospice
(Greensboro & Burlington, NC) – AuthoraCare Collective
Job Summary Manage the overall plan of care for a caseload of hospice patients in collaboration with the interdisciplinary team. Assess and identify patient/caregiver problems, establish nursing goals and interventions, and work with physicians to develop a medical plan of care. Build and maintain a therapeutic relationship with patients and caregivers, provide emotional support, education, and dietary counseling as needed. Communicate effectively with all team members and document outcomes. Assume on‑call responsibilities as scheduled.
Responsibilities
Collaborate with interdisciplinary group (IDG) to coordinate care plans.
Assess patient and caregiver needs and set nursing goals.
Implement interventions and monitor progress.
Work with physicians to establish and update medical care plans.
Provide emotional support, teaching, and dietary counseling.
Document care outcomes accurately.
Assume on‑call responsibilities as scheduled.
Requirements
Graduate of an accredited School of Nursing.
Current RN/LPN license in North Carolina with good standing in NCBON.
CPR certification from the American Heart Association or the American Red Cross Basic Life Support (BLS) (required).
Experience in an acute, hospice, home health, or long‑term care setting (preferred).
Valid state‑issued driver’s license.
Own transportation and automobile liability insurance at agency‑required limits.
