Location Clovis, NM
Salary $1,800 - $11,500 per month (based on monthly average of 20–100 visits, $90–115 per assessment)

Responsibilities
Conduct adult/geriatric assessments, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation according to Matrix guidelines and protocols.
Administer and order point‑of‑care and lab‑based testing as needed.
Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
Collaborate with Primary Care Physician (PCP) on patient education and follow‑up as necessary.
Provide services at the member’s place of residence, such as their homes or Skilled Nursing Facilities.

Qualifications
Master’s Degree or equivalent experience and successful completion of NP licensure.
Current NP licensure in the state of practice, including prescription authority or ability to obtain it.
AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
Current BLS, ACLS, or CPR certification.
One year of NP experience preferred; new graduates encouraged.
Valid state driver’s license and proof of adequate automobile insurance coverage.
Strong computer skills and familiarity with employee health/medical record software.
Excellent verbal and written communication skills.
