Responsibilities
Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
Administer and order point of care and lab-based testing as needed
Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary
Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities
Benefits
Flexibility - Visits are scheduled based on YOUR availability. Assessments may be conducted Sundays through Saturdays, with options across day, evening, and weekend shifts.

Qualifications
Master’s Degree or commensurate experience with NP licensure
Current NP licensure in state of practice with prescriptive authority or ability to obtain
AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care
Current BLS, ACLS or CPR certification
1 year of NP experience preferred; new grads encouraged to apply
Valid state driver’s license and proof of auto insurance
Strong computer skills and familiarity with medical record software
Excellent verbal and written communication skills
