RN for Home Health -PRN Agency in Wayne County Job Summary: We are seeking a Registered Nurse (RN) to join our Home Health team on a PRN (as needed) basis in Wayne County. The RN will provide compassionate and high-quality care to patients in the comfort of their homes, ensuring their well‑being and promoting independence.
Qualifications:
Valid RN license in the state of Michigan
Minimum of 1 year of nursing experience, preferably in home health or a similar setting
Strong clinical skills and the ability to assess, plan, implement, and evaluate patient care
Excellent communication and interpersonal skills
Ability to work independently and as part of a multidisciplinary team
Reliable transportation and a valid driver's license
CPR certification
Responsibilities:
Conduct home health assessments and develop personalized care plans for patients
Administer medications, treatments, and procedures as prescribed by the physician
Monitor patient progress and report any changes to the healthcare team
Provide patient and family education on disease management and self‑care techniques
Coordinate with other healthcare professionals to ensure holistic care for patients
Maintain accurate and timely documentation of patient care
