Job Summary
APC – Primary Care at Home
– Kelsey‑Seybold Clinic
This role works under the supervision of the Managing Physician for Primary Care at Home to provide value‑based medical care and care coordination for homebound patients. The Care Manager – Nurse Practitioner will assess patients, perform comprehensive or problem‑focused examinations, order and interpret laboratory and diagnostic tests, prescribe and manage therapeutic treatments, and develop comprehensive, outcomes‑based patient care plans. The Care Manager will also initiate timely consultations and referrals when patient problems exceed their scope of practice. This position requires home and virtual visits; the goal is to perform about six home visits per day.
Responsibilities
Serve as physician extender in the clinic setting
Evaluate patients in the clinic setting
Document evaluations in a timely manner
Order medications and tests as per guidelines and protocols
Follow up on tests and consults
Schedule clinical studies as per guidelines and protocols
Coordinate referrals/consults
Handle MKO messages, results, CC charts, telephone encounters, and surgical risk assessments as needed within scope of practice and as directed by the supervising physician
Qualifications
Graduate from an Accredited School of Physician Assistant Studies or Master’s in Nursing
Licensed in the State of Texas
Board Certified and/or Board Eligible
2 years of experience in an outpatient primary care setting required
