Job Summary This position reports to the Managing Physician for Primary Care at Home and provides value‑based medical care and coordination for homebound patients. The Nurse Practitioner Care Manager assesses patients’ physical and psychosocial conditions, orders and interprets diagnostics, prescribes treatments according to protocols, and implements comprehensive outcomes‑based care plans.

Responsibilities

Serve as physician extender in the clinic setting

Evaluate patients in the clinic setting

Document evaluations in a timely manner

Order medications and tests as per guidelines and protocols

Follow‑up on tests and consults

Schedule clinical studies per guidelines and protocols

Coordinate referrals and consults

Handle MKO messages, results, CC charts, telephone encounters, and surgical risk assessments within scope of practice and as directed by the supervising physician

Required Qualifications

Graduate from an accredited School of Physician Assistant Studies or hold a Master’s in Nursing

Licensed in the State of Texas

Board Certified and/or Board Eligible

Two years of experience in an outpatient primary‑care setting

Compensation for this specialty ranges from $104,500 to $156,000. Total cash compensation includes base pay and bonus and may increase over time based on productivity and performance.
