Job Summary: This position works under the supervision of the Managing Physician for Primary Care at Home to provide value-based medical care and care coordination for homebound patients. The APC is involved in planning, implementing, and evaluating medical and nursing care for patients with complex care needs. The Care Manager – Nurse Practitioner assesses physical and psycho-social conditions of patients by performing comprehensive or problem-focused physical examinations and medical histories. The Care Manager orders, collects, interprets, and evaluates laboratory and other diagnostic tests to assess patient problems and health care needs. The Care Manager may prescribe and/or recommend medical drugs or other treatments in accordance with approved protocols and policies, with attention to efficacy, safety, and cost. The Care Manager formulates and implements comprehensive outcomes-based patient care plans based on patient assessments. Note:

Responsibilities: Serve as physician extender in the clinic setting Evaluate patients in the clinic setting Document evaluations in a timely manner Order medications and tests per guidelines and protocols Follow up on tests and consults Schedule clinical studies per guidelines and protocols Coordinate referrals/consults Handle MKO messages, results, CC charts, telephone encounters and surgical risk assessments as needed within scope of practice and as directed by the supervising physician Qualifications: Graduate from an Accredited School of Physician Assistant Studies or Master’s in Nursing Licensed in the State of Texas Board Certified and/or Board Eligible 2 years of experience in an outpatient primary care setting required Compensation and Benefits: Compensation generally ranges from $104,500 to $156,000. Total cash compensation includes base pay and bonus and is based on factors including local labor markets, education, work experience, and may increase over time based on productivity and performance. Benefits include a comprehensive package, incentive and recognition programs, equity stock purchase, and 401k contribution (eligibility requirements apply). A wide range of benefits and incentives is available based on location and role.
