Per Diem Pharmacist – Westminster Communities of Florida (Orlando, FL)
Join a dedicated team where pharmacists evaluate medication orders for appropriateness, dosage, potential interactions, route of administration, and stability. Position requires availability on nights and weekends (5pm‑11pm). This role honors those who serve. EOE, DFWP
Responsibilities
Make recommendations for therapy changes to physicians and facility staff.
Conduct Drug Utilization Review (DUR) on all prescriptions and review clinical edits to ensure no adverse drug interaction.
Maintain patient/resident database with current and past medical history, laboratory data, and concurrent medication (including OTC and home remedies).
Work with pharmacy technician to reconcile Controlled Substances Log(s) with inventory and prescriptions dispensed, complying with state and federal regulations.
Provide clinical expertise by responding to drug-related questions and concerns from internal and external customers.
Document all communications in the medical record.
Schedule may include weekend and/or nights.
Minimum Qualifications
Doctorate of Pharmacy (PharmD).
One to three years long‑term care (LTC) pharmacy experience.
Current licensure in the State of Florida.
IV clinical experience preferred.
Strong organizational skills and ability to prioritize and manage multiple tasks in a dynamic environment.
Per Diem staff must work at least one shift per month.
