Base pay range $21.00/hr - $23.00/hr
Case Manager - Children's Behavioral Health:
JFCS is seeking a compassionate professional to provide case‑management services to children receiving behavioral‑health services across The Valley. The Child Case Manager also provides guidance and support to the families/caregivers of these youth as needed.
Duties include:
Provides weekly contact with assigned clients, which includes home/community visits, attending medication checks, etc.
Assists youth in navigating discharge from treatment settings, in finding services as clinically indicated, and working to decrease behaviors to improve stability in the home & community.
Coordinates services and collaborates with other involved agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
Coordinates and facilitates Child & Family Team (CFT) Meetings.
Collaborates with clinical team to ensure that Integrated Service Plans, CFT Meeting notes, and all required paperwork is completed accurately and in a timely manner.
Qualifications:
Meets one of the following education/experience requirements:
High School Diploma/GED required and 4 years of Behavioral Health experience.
A.A. Degree and two years of Behavioral Health experience
Bachelor’s Degree in Social Work, Psychology, or related field.
Level 1 Fingerprint Clearance Card without Restriction(s)
Meet Agency requirements for MVR (Motor Vehicle Record) report
Reliable transportation
AZ driver’s license
