Job Overview
Matrix Medical Network is hiring a PRN Nurse Practitioner to conduct comprehensive health assessments at members’ homes and skilled nursing facilities. Responsibilities
Conduct adult/geriatric assessments including history, diagnosis, treatment, education, and referrals per Matrix guidelines. Administer point of care tests and order lab-based testing as needed. Collaborate with physicians, case managers, social workers, and ancillary staff to coordinate care. Educate patients and families and collaborate with the Primary Care Physician on follow‑up. Provide services at the member’s residence or Skilled Nursing Facility. Qualifications
Master’s Degree or commensurate experience with NP licensure in state of practice. Current NP licensure with prescription authority or ability to obtain prescriptive authority. Board certification in an adult primary care specialty (e.g., AANP, AACN, ANCC). Current BLS, ACLS or CPR certification. 1 year of NP experience preferred; new grads encouraged. Valid state driver’s license and proof of adequate automobile insurance. Strong computer skills and familiarity with medical record software. Excellent verbal and written communication skills. Flexible scheduling—visits scheduled based on availability from 8 am‑7 pm Sunday‑Saturday. Competitive pay—$90–$115 per assessment, $1,800–$11,500 per month based on volume.
