Hospice Admissions RN role offering a $15,000 hire bonus, pension, work schedule M – F 11 AM to 7 PM, NO weekends. Hosted by Donna Gawroski‑Kusik.
We are a NYC Metro Healthcare organization with over 100 years of success, offering hospice, palliative care, rehabilitation, and skilled nursing services.

Responsibilities
Act as the first point of contact for patients and families considering or entering hospice care.
Conduct thorough assessments to ensure hospice is the appropriate level of care, explain hospice services and philosophy, and coordinate the admission process, including medication and equipment needs.
Work as part of an interdisciplinary team to ensure a smooth transition and provide comfort‑focused care.
Visit patients in SNF, rehab, and hospital settings in the Bronx and Queens.
Coordinate admission: collect necessary patient information, including medical history, insurance details, and consent forms.
Reconcile medications, ensuring all are accounted for, reconciled, and ready for the patient.
Create and implement the initial plan of care in collaboration with patient, family, and physician.
Ensure all necessary forms are signed, including consent forms and advanced directives (e.g., DNR).
Collaborate with physicians, social workers, chaplains, and other hospice team members to provide comprehensive care.
Provide ongoing communication and support to the patient and family throughout the admission process.
Meticulously document all assessments, interventions, and communications in the patient’s record.
Transfer information to the designated case manager after initial admission is complete.
Support the patient and family during the transition into ongoing hospice care.
