Associate or Independently Licensed Counselor
to meet the high demand of our behavioral health services at our Glendale, AZ location.

Clinical Supervision provided in-house
Responsibilities
Complete initial and ongoing assessments of clients assigned to your panel
Provide individual, group, couples, and family counseling
Maintain client-related paperwork (diagnosis bill, assessments, progress notes, and other required forms)
Evaluate physical and/or mental conditions based on review of information and observation of the client with the integrated treatment team
Refer members to community resources and internal services as necessary and engage in population health
Maintain confidentiality of records aligning with HIPAA requirements
Assess for risk of harm to self and others and provide evidence-based measures to enhance client outcomes
As an independently licensed clinician, conduct reviews and sign off on assessments, service plans, and other BHP requirements of associate level staff
Requirements
Master’s degree in counseling, social work, or related field
Active License in the state of Arizona (LCSW, LPC, LMFT, LMSW, LAC)
Independently Licensed clinician (preferred)
Current CPR, AED, First Aid certifications (required)
Current DPS Level One Fingerprint Clearance Card (required)
Board Authorized Clinical Supervisor (preferred)
Bilingual (preferred)
Knowledge of Microsoft Software especially 365 platform
Experience with electronic health records (EHR); Advanced MD (preferred)
Strong organizational skills and communication skills
Knowledge of integrated healthcare delivery models
