Reporting to the Director of Clinical Services, this role ensures the accuracy, compliance, and quality of hospice assessments by reviewing for regulatory and policy adherence. The individual provides clinical staff with guidance on documentation, utilization, and patient‑care decisions to support optimal clinical and operational outcomes. The role reviews visit utilization for care appropriateness, collaborates with clinicians on necessary modifications, and reports potential over‑ or under‑utilization to leadership. Timely completion of workflows in Homecare Homebase (HCHB) and flexibility to meet agency needs are essential. Collaboration with teams is key to identifying trends and developing action plans. Participation in quality and performance‑improvement initiatives is expected, along with supporting compliance with data‑submission requirements for regulatory and payer standards.
Key responsibilities include:
Reviews hospice assessments for accuracy, completeness, and regulatory compliance, providing recommendations to ensure quality and reliability.
Reviews visit utilization for appropriate patient care, collaborates and consults clinicians on needed adjustments, and reports utilization concerns and financial risks to the clinical manager and director of clinical services.
Ensures timely completion of HCHB workflows and maintains a flexible schedule to support agency needs.
Collaborates to identify documentation trends, supports improvement planning, participates in quality initiatives, and helps ensure compliance and accurate data submissions across all settings.
Guides staff on clinical, documentation, and utilization decisions to improve accuracy and care quality, recommending cost‑effective corrections that support clinical and operational goals.
Qualifications
Education: Minimum – Degree from an educational program with licensure as a Registered Nurse (RN); Preferred – Bachelor’s degree in nursing.
Experience: Minimum – 2 years in a hospice clinical role; Preferred – 3 years in a hospice clinical role with a focus on quality & compliance, and Homecare Homebase experience.
Licensure Required: Current Connecticut RN license.
