Program Engagement Coordinator
The Program Engagement Coordinator is responsible for service coordination and outreach. The Program Engagement Coordinator supports participant engagement from referral through active enrollment by managing intake, coordinating care, and ensuring seamless communication among the interdisciplinary team and external partners. The Program Engagement Coordinator also contributes to strategic growth by cultivating referral relationships, attending community events, and assisting with initiatives to expand services and partnerships.
Responsibilities
Process referrals and intake assessments, coordinating the admission process with the multi-disciplinary team and other service providers
Inform participants of, and refer participants to, available community services
Participate in the completion of comprehensive assessments and plans of care
Document service coordination notes in the participant's electronic health record upon service commencement, when significant changes occur, and at least one time every 90 days or more often as necessary to ensure the health record reflects the current clinical needs of the participant
Provide coordination of the medically-related physical, mental, and behavioral, service needs of a participant at the time of enrollment, during treatment and care in the Program, or at the time of discharge
Assist with any personal, social, family, or adjustment problems the participant may experience at the Program
Provide referrals for the participant or family to the appropriate community resources if the participant or the participant's family requires specialized counseling
Coordinate services if a participant needs services from other community agencies, and no agency is acting as coordinator of services for that participant
Assess, plan, implement, coordinate, monitor, and evaluate the services required to meet a participant's needs.
Provide direct care assistance as needed, including assistance with ADL's such as toileting, bathing, and feeding
Contributes to agency-wide projects to expand services and community partnerships; assists the Chief Strategy Director in strategic planning initiatives.
Responsible for outreach and collaboration with client referral sources to ensure lead generation
Responsible for attending events in the community such as job fairs, resources fairs, and more
Responsible for daily data entry in electronic records, ensuring documentation is accurate and complete
Other duties as assigned by supervisory staff.
Equipment
Medical and participant care equipment as needed to meet the needs of Adult Day Health Center's participants.
Working Conditions
Indoors.
Extensive participant contact.
Possible exposure to infection, blood and blood borne pathogens.
May work with hazardous/infectious waste.
Qualifications & Skills
Bachelor's degree in social work or related field required
Minimum 2 years' experience working with the aging population
Computer proficiency required
Current driver's license and access to reliable transportation required
Salary starting at $57,000
