Job Summary

The Quality Assurance Specialist II (QM Specialist) performs complex (journey-level) quality management work. Work involves assisting with planning, developing, and implementing departmental and hospital-wide QM programs to include policy, QSO (Quality System Oversight) monitoring, Restraint/Seclusion processes, and other performance improvement activities. Ensures compliance with The Joint Commission (TJC) and Centers for Medicaid and Medicare Services (CMS). Works with department and agency staff as well as external stakeholders. Provides guidance to others, as requested. Works under general supervision, with moderate latitude for the use of initiative and independent judgement. Performs other duties as assigned, including participation in disaster response and/or recovery or Continuity of Operations (COOP) activation, which may require an alternate shift pattern or location.

Responsibilities

Plan, develop, and implement departmental and hospital-wide QM programs including policy, QSO monitoring, restraint/seclusion processes, and performance improvement activities.

Ensure compliance with TJC and CMS regulations.

Collaborate with department, agency staff, and external stakeholders; provide guidance and education on organizational/statutory requirements and accreditation standards.

Coordinate quality improvement and monitoring programs (e.g., QSO, Utilization Management, FSPI) and independently complete all aspects of this process.

Compile monthly samples, updates, and distribute audits; provide education and support to auditors.

Aggregate data, compile reports, and conduct trend analysis; communicate regularly with the corresponding Central Administration program coordinator.

Serve as data oversight coordinator for QAPI indicators such as restraint/seclusion, injuries/falls, Length of Stay (LOS), national benchmarks, Patient Tracers, etc.; monitor documentation of incidents.

Aggregate data, coordinate committee meetings, and compile agenda documents and reports for hospital committees, Governing Body, and Quality Management Director.

Oversee submission of reports to the Hospital Executive Committee and to the appropriate committees of the State Hospital System Governing Body.

Prepare, edit, and distribute correspondence, reports, and documents; maintain filing and records management systems.

Complete other audit assignments as needed (e.g., TJC monitoring, performance improvement projects).

Assist in disaster response, recovery, or COOP activation as required.

Knowledge, Skills & Abilities

Knowledge of local, state, and federal laws related to the program area.

Knowledge of TJC and CMS standards.

Knowledge of statistical analysis processes, research techniques, training techniques, and project management processes.

Skills in interviewing and analyzing data.

Proficiency in computer use and applicable software.

Ability to plan, coordinate, and administer quality improvement programs.

Ability to identify problems and develop solutions.

Ability to communicate effectively.

Ability to provide guidance to others.

Initial Screening Criteria

Two years of experience in social services, psychiatric/health care, auditing or investigating work; and developing plans of correction.

Graduation from an accredited four-year college or university with major coursework in social services, nursing, psychology, or a related field; minimum of six months experience required (preferred).

Associate degree and a minimum of twenty-four months experience required.

No degree and a minimum of four years of experience required.

Additional Information

Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aid, Medication Aide and Employee Misconduct, List of Excluded Individuals/Entities (LEIE). Males between the ages of 18-25 must be registered with the Selective Service. All applicants must be at least 18 years of age to be considered for employment at a state-operated facility; Waco Center for Youth applicants must be 21 years of age. Flexibility in work hours may be required; overtime and/or extended hours may be required. Compliance with HHSC immunization policy and state hospital operating procedures related to immunizations is required (vaccines may be provided). The position may require wear of a mask and other protective measures if not immunized for influenza. Applicant may be required to undergo background checks and work eligibility verification. Telework is not available for this position.
