Salary range: $98,000.00 - $130,000.00 per year.
Job Requirements
Have at least 2-3 years of experience in the primary care/value-based care setting, working with
Medicare Advantage ,
Medicare , and Medicaid patient populations
Must have an active and unrestrictive Texas APRN license
Must have DEA license or be eligible to obtain it within 2 months of hire date
Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
Excellent documentation and patient relationship-building skills
Proficient with EMR systems
