Job Overview Schedule: Monday-Friday (8AM-4:30PM) or (8:30AM-5PM) Are you passionate about shaping the health and development of young children? At Legacy Community Health, we’re looking for a dedicated Patient Educator with a special focus on early childhood screening and development. In this role, you’ll be a vital part of our pediatric care team, working directly with families to support children’s growth, identify developmental milestones, and promote lifelong wellness from the very beginning.
Role Highlights:
Use motivational interviewing and goal-setting techniques to empower families to support their child’s development.
Collaborative Environment:
Partner with pediatricians, behavioral health specialists, and community resources to ensure comprehensive care for children.

Conduct developmental screenings and provide education to caregivers on early milestones, nutrition, and behavioral health.

Key Responsibilities
Deliver health education programs in individual, co-visit, and group settings with a focus on early childhood development.
Conduct screenings for developmental delays, social-emotional health, and school readiness using validated tools.
Support caregivers in setting developmentally appropriate goals and navigating early intervention services.
Document all interactions in electronic health records and contribute to data tracking for early childhood outcomes.
Develop and maintain educational materials for families on topics such as speech/language development, motor skills, and positive parenting.
Participate in interdisciplinary team meetings to coordinate care and share insights on child development trends.
Contribute to grant‑funded initiatives and performance metrics related to early childhood health.
Minimum Qualifications
High School Diploma or equivalent required.
3–5 years of relevant experience; experience in early childhood education, pediatrics, or child development preferred.
Bachelor’s Degree in a related field with at least 1 year of relevant experience preferred.
Experience working with families from diverse cultural backgrounds in one‑on‑one and group settings.
Familiarity with developmental screening tools (e.g., ASQ, M‑CHAT) is a plus.
Bilingual in English and Spanish strongly preferred.
Ability to complete point‑of‑care testing and phlebotomy training within 3 months of hire, if required.
