SPEECH LANGUAGE PATHOLOGIST (SLP) – WOODHAVEN

as an experienced Speech Language Pathologist to work with a multidisciplinary team to provide individualized therapy plans for patients in a supportive rehabilitation setting.
Responsibilities
Collaborate with physicians, nurses, and other therapists to develop and implement comprehensive treatment plans.
Deliver in‑house speech therapy using progressive modalities tailored to various disease states and impairments.
Maintain accurate documentation and communication with staff, patients, and families regarding progress and outcomes.
Participate in team meetings and contribute to continuous quality improvement initiatives.
Requirements
Education:
Graduate of an accredited therapy (Speech Pathology) program.
Certification / License:
Must be licensed in the state where the Skilled Nursing Facility (SNF) is located and possess three C’s.
Experience:
Prior SNF or rehabilitation experience preferred; 1 year experience preferred.
No CFY (Career in the Workforce) position.
Background checks/drug‑free workplace. EOE.
