Perform psychosocial assessments and develop individualized care plans. Provide counseling to residents and families and advocate for residents’ rights and needs. Facilitate communication between residents, families, and staff. Coordinate discharge planning and ensure compliance with regulations. Assess needs, plan, and manage all aspects of the comprehensive activities program. Get to know each resident individually to better understand needs and interests. Complete screens and orientate residents and families to Oak Park Place. Help residents and families adjust to the center. Assist families and residents with financial questions and referrals. Qualifications
Associate degree preferred. Previous experience in a supervisory role. Previous experience working with seniors is preferred. Strong organizational and planning skills required. Ability to work independently is a must. Benefits
