Responsibilities
Provide thoughtful care to residents based on personalized Plane of Care.
Monitor and track changes in residents' conditions and needs.
Proactively update the appropriate contacts regarding residents’ plane of care, health changes, and concerns.
Requirements
Previous experience in nursing or other medical fields.
Familiarity with medical software and equipment.
Ability to build rapport with patients.
Strong problem solving and critical thinking skills.
Ability to thrive in a fast‑paced environment.
Current RN license.
