Job Summary and Qualifications The Inpatient Rehab Facility Patient Assessment Instrument Coordinator functions under the Rehab Program Director. This role provides coordination for the timely and accurate completion of the Patient Assessment Instrument for each rehabilitation inpatient and offers clinical support in the completion of data contained in the PAI to ensure accuracy in the scoring of functional status and reflection of the patient’s care needs. The coordinator may also assist in the ongoing monitoring of identified benchmarks for the inpatient population, maintain confidentiality of all patient information and collaborate with hospital coders and Rehab physician staff in the appropriate identification and documentation of co‑morbid conditions.
Essential Job Functions:
Research clinical documentation for evidence of co‑morbid conditions and verify accuracy of coding under the direction of the attending Rehab physician.
Educate staff to improve FIM score accuracy, review clinical documentation, recommend improvements to clinical supervisors and serve as an information source and educator for IRF‑PAI users. Serve as the FIM credentialing coordinator to ensure facility FIM compliance.
Maintain current knowledge of the PAI software and provide expertise on this software within the inpatient rehab setting. Keep updated and ongoing knowledge of the regulations concerning the PAI and its completion within the inpatient rehab setting. Oversee completion and ensure accuracy and the completion of the Inpatient Rehabilitation Facility‑Patient Assessment Instrument (IRF‑PAI) prior to electronic transmissions.
Verify appropriate CMG designation on final UB‑04 through chart review, staff interview and collaboration with the Rehab Medical Director. Provide ongoing education to the hospital staff to ensure accurate and timely completion of the PAI.
Coordinate the completion of the admission assessment of the PAI by the fourth (4th) day of the patient’s stay and the fifth (5th) day following discharge for each inpatient rehabilitation patient. May require discussion/interactions with various staff members concerning differences in reported functional status to complete the PAI. Submit completed IRF PAI to HIM Department for coding transmittal. Periodically audit PAI, clinical documentation and billing information to ensure accuracy and consistency of documentation. Work closely with all members of the clinical team to ensure compliance on all levels and demonstrate effective critical thinking skills.

Collaborate with physicians and interdisciplinary team members to make the most accurate assessment of the needs and strengths of the patients.
What qualifications you will need: Minimum Education, Experience, Training, Required Knowledge, Skill, Abilities and Other Job-Related Characteristics:
Education: Licensed clinician, such as RN, PT, OT, ST.
Experience: Two or more years in an acute rehabilitation setting.
Employee completes initial and ongoing training and competencies as defined by service line, facility and department/unit which are specific to the needs of the patient population served (if applicable).
Preferred experience: RN, CRRN clinical experience in an acute hospital or rehabilitation setting; case management related experience in inpatient rehab unit; previous IRF PAI experience; coding experience.
Minimum License, Certificates, or Registrations Required:
Current Texas License as applicable for profession.
FIM credentialed within 90 days of hire.
