The interdisciplinary team focuses on supporting independence, dignity, and quality of life for seniors who might otherwise require nursing home care.
Opportunity:
Nurse Practitioner - Geriatric Medicine
Job Details:
• Full-time position with an interdisciplinary PACE team providing comprehensive care to frail elderly adults• Perform comprehensive medical assessments for program participants• Develop and implement individualized care plans for seniors with chronic needs• Provide acute illness evaluation and treatment as needed• Participate actively in discharge planning processes• Conduct home visits when clinically indicated• Coordinate care across the full continuum of services offered by PACE• Assist in the development and review of clinical policies and procedures• Participate in provider on-call rotation• Must be a Board Certified Nurse Practitioner (ANCC or AANP)• Requires active Massachusetts NP license, DEA Registration, and CDS Certification• BLS Certification required• Must have experience with frail or elderly population• Ability to travel within service area is essential
