Join our interdisciplinary PACE team in West Springfield, Massachusetts, and provide comprehensive, community-based care to frail and elderly adults while supporting their independence, dignity, and quality of life.
Our program serves individuals who are 55 or older and are certified to require nursing home care, enabling them to live safely in their communities. We deliver a full continuum of medical and supportive services to seniors with chronic needs while ensuring they maintain their independence for as long as possible.
Job Responsibilities:- Perform comprehensive medical assessments
- Develop and implement individualized care plans
- Provide acute illness evaluation and treatment
- Participate actively in discharge planning
- Conduct home visits as clinically indicated
- Coordinate care across the full continuum of services offered by PACE
- Assist in the development and review of clinical policies and procedures
- Participate in provider on-call rotation
- Board Certified Nurse Practitioner (ANCC or AANP) REQUIRED
- Active Massachusetts NP license REQUIRED
- DEA Registration REQUIRED
- CDS Certification REQUIRED
- BLS Certification REQUIRED
- Experience with frail or elderly population REQUIRED
- Ability to travel within service area REQUIRED
- Nurse Practitioner certification with prescriptive authority
- Board Certified Nurse Practitioner (AANP or ANCC)
- Active Massachusetts NP license
- DEA Registration
- CDS Certification
