Provider participates in on-call rotation
Home visits as clinically indicated
Perform comprehensive medical assessments
Develop and manage individualized care plans
Evaluate and treat acute and chronic conditions
Coordinate care across the full continuum of services
Participate in discharge planning
Conduct home visits within the service area
Collaborate with an interdisciplinary care team
Support clinical policy development and quality initiatives

Board Certified Nurse Practitioner (AANP or ANCC)
Active Massachusetts NP license with prescriptive authority
DEA & CDS
BLS certification
