Public/Community Health Nurse Practitioner

Specialty: Physical Health/Behavioral Health (FNP or PA preferred, PMHNP considered only if they are very comfortable with physical health and feel it s firmly within their scope)

• Start Date: February 2025
• Contract Length: TBD but I am thinking 3 months (covering a maternity leave)
• Shift Details: 8a-5p
• Schedule: Monday and Friday, weekly

Work Details:

• Support staff: One MA and one RN. A therapist and case manager are also available to help with referrals, therapy and other client needs
  
• Patients per day: 12 pts per day
• Patient population/age: Clinic located within a homeless shelter - Co-occurring behavioral health and physical health needs
• Common Clinical Focus: Foot care, wound care, Infectious Disease
• Skills Required: Needs to be able to do physical medicine, some wound management, manage behavioral health and substance use (sometime active intoxication)

Requirements:

• Board Certification: FNP preferred, PMHNP considered only if they are very comfortable with physical health and feel it s firmly within their scope
  
• Credentialing Timeframe: Outpatient, very fast credentialing
