Summary of Job Duties:

The Registered Nurse will promote wellness, improve outcomes, and self‑care within a defined population of patients. The RN will provide patients with chronic conditions with the support, education, and assistance necessary for the prevention and/or maintenance of their disease and/or health and wellness state. Will work with the individual patient, their families, and providers of care to develop and implement individual health plans and provide support to the patient in self‑management.

Minimum Qualifications:

Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing

Valid RN License

Minimum of 2 year Registered Nursing experience

Preferred Qualifications:

Three (3) years of RN experience with two (2) years of experience in a clinical environment

Current BLS certification or must successfully obtain certification within the first 30 days of employment

Responsibilities:

Collects and documents assessment data from patients and/or others per UAMS standard.

Develops patient's Plan of Care (POC) per department/unit standards utilizing assessment data.

Evaluates the progress of patients toward attainment of desired outcomes.

Administers medication following department/unit standards and Nurse Practice Act.

Verifies and evaluates care delivered to ensure it is consistent with patient and family centered care principles, practice standards and regulatory requirements.

Supports the integration of current evidence into nursing practices.

Promotes an environment conducive to positive educational experience for all students and staff.

Provides effective patient/family education/discharge planning.

Effectively collaborates and communicates with nursing staff at all levels, interdisciplinary teams, executive leaders, and other stakeholders.

Communicates care appropriately through documentation in the medical record.

Reports variances in care/ systems/ processes through the appropriate means.

Participates in the recruitment/retention of staff.

Facilitates the development and implementation of evidence‑based nursing practice standards that comply with ANA standards.

Coordinate care and/or follow‑up for patients who receive care in outpatient facilities. Coordination of prescription refill requests and patient education coordination in collaboration with the physician staff.

Commitment to patient outcomes results in high levels of performance despite workload changes.

May perform other duties as assigned.

Salary Information:

Commensurate with education and experience.
