Registered Nurse Case Manager – Hospice of Chattanooga, Chattanooga, TN
Overview We are looking for a high-energy, compassionate, detail-oriented Registered Nurse (RN) to serve as a Hospice Case Manager. The RN Case Manager plans, organizes, and directs hospice care utilizing the nursing process, and effectively interacts with patients, caregivers, families, and interdisciplinary team members while maintaining professional nursing standards.
Responsibilities
Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
Partner closely with the Clinical Director to assess eligibility and suitability of clients for hospice services.
Coordinate direct care, ensuring patient comfort and quality of life remain paramount.
Deliver top‑notch nursing care rooted in proven principles and techniques.
Educate and empower patients and families, encouraging participation in personalized care plans.
Participate in call duties, ensuring timely support when needed.
Qualifications
Current, unencumbered RN license.
Minimum one (1) year of RN experience in a medical‑surgical or acute care setting; hospice experience a plus.
Computer proficiency, including experience with EMR systems.
Current CPR certification, if required by state.
Compensation $31.00 – $36.00 per hour. Final compensation will be determined based on experience, qualifications, and responsibilities and confirmed at the conclusion of the interview process.
