PRAIRIELAND SERVICE COORDINATION, INC.
Prairieland is seeking a Case Manager/QIDP to serve Peoria County. A bachelor’s degree in a human services related field and one‑year cumulative experience providing direct service to individuals with developmental disabilities (employment, volunteer or family care) are required. The position offers excellent benefits, an 8:00 a.m. – 4:30 p.m. schedule Monday through Friday, and flexibility to work partially from home.
General Description Under the direct supervision of the Program Manager of Prairieland Service Coordination, Inc., the Case Manager assesses individual needs and ensures the provision of necessary services through communication, service coordination, linkage with provider agencies, service monitoring, and individual advocacy.
Job Responsibilities
Prairieland Service Coordination Inc. provides services in twenty‑three counties and a Case Manager may be requested to work in a county outside their typical geographic area (e.g., staff illness or resignation).
Complete the PAS process following the PAS manual procedures.
Advocate on behalf of the individual and/or family to assist in obtaining services to which they are eligible and have chosen, ensuring direct involvement and participation of the individual and their family in this process.
Review individual's medical, residential, vocational, educational, legal, and financial status needs and choices through Visiting Notes as required.
Schedule and complete Discovery and Personal Plan processes annually, or as required.
Maintain individual files as specified by program and agency guidelines.
Provide problem‑solving consultation with provider agencies following the DHS Conflict Resolution guideline.
Coordinate specialized services for eligible Bogard individuals as needed and submit summary reports to external agencies as appropriate.
Monitor services provided to Bogard class members on a monthly basis and advocate for any unmet needs.
Enter billings daily into Birdseye to ensure all time reporting for payroll is submitted on time and accurately.
Participate in scheduled all staff meetings and mandated training sessions as recommended by the Executive Director and/or Program Manager.
Alert the Program Manager of PSCI to problematic issues, trends or service gaps.
Perform other related work as required or requested by the Executive Director and/or Program Manager.

Job Qualifications
A bachelor's degree in human services and QIDP eligibility as established by the Department of Human Services.
Ability to organize, plan and follow a schedule on a daily, weekly, monthly and annual basis.
Demonstrated working knowledge of the developmental disabilities service delivery system, including recipient eligibility requirements and procedures.
Effective interaction with professionals, paraprofessionals, social service organizations, and clients including their families.
Ability to work well in teams and in a remote home office environment using self‑discipline and self‑motivation, with minimal supervision.
Flexibility to work hours outside the normal workweek.
Willingness to follow directions, accept constructive criticism, and accept responsibility for own actions.
Pre‑existing adherence to the Employee Code of Ethics Guideline through professional use of oral and written communication.
Reliable transportation, valid Illinois driver’s license, personal auto insurance, and a good motor vehicle record.
