We are looking to add a Case Manager/QIDP to serve Moultrie, Douglas, and Coles counties in IL. A bachelor’s degree in a human service related field and one year cumulative experience providing direct service to individuals with developmental disabilities are required. The role schedule is 8:00 am to 4:30 pm, Monday through Friday, with flexibility to work partially from home.

General Job Description Under the direct supervision of the Program Manager, the Individual Service Coordinator assesses individual needs and assures the provision of necessary services through communication, service coordination linkage with provider agencies, service monitoring and individual advocacy.

Job Responsibilities

Provides services in twenty-three counties and may be requested outside geographic area.

Schedules and conducts intake/PUNS interview in response to referrals, gathering initial information as required under the PAS Manual and conducts annual reviews as needed.

When eligible, completes the PAS process following designated procedures, utilizing assistance from supervisor.

After initial assessment, provides linkage to other community services for individuals ineligible for PAS/Individual Service Coordination services.

Advocates on behalf of the individual and/or family to obtain eligible services and ensures direct involvement of individual and family.

Reviews individual's medical, residential, vocational, educational, legal and financial needs through ISC Visiting Notes as required.

Schedules and completes Discovery and Person Centered Plan process annually or as required.

Accurately documents services and maintains individual files as specified by program and agency guidelines.

Functions in problem solving capacity with provider agencies regarding service coordination, following ISC Conflict Resolution guideline.

See all waiver individuals as required and complete program requirements set forth in waiver manual.

Coordinates specialized services for eligible Bogard individuals and submits summary reports to external agencies.

Monitors services for Bogard class members monthly and advocates for unmet needs.

Submits all required documents to Program Manager for payroll preparation.

Participates in scheduled staff meetings and mandatory training sessions as recommended.

Alerts the Program Assistant of PSCI to problematic issues, trends or service gaps.

Performs other related work as required by the Executive Director and/or Program Manager.

Job Qualifications

Bachelor's degree in human services required; QIDP eligibility established by Department of Human Services.

Ability to organize, plan and follow a schedule daily, weekly, monthly and annually.

Working knowledge of developmental disabilities service delivery system, including eligibility requirements and procedures.

Effective interaction with professionals, paraprofessionals, social service organizations and clients and their families.

Ability to work well in teams and at a home office using self‑discipline and self‑motivation without direct supervision.

Flexibility to work outside normal workweek hours.

Cooperative teamwork toward mutual program goals.

Willingness to follow directions, accept constructive criticism and accept responsibility for actions.

Professional presentation and communication (oral and written) in line with Employee Code of Ethics.

Reliable transportation; valid Illinois Driver's License and auto insurance with good motor vehicle record.

Physical Expectations

Hourly Wage Range: $19.95 - $21.63 per hour
