PRAIRIELAND SERVICE COORDINATION, INC.
We are looking to add to our Prairieland family in Galesburg, IL. Prairieland is seeking a Case Manager/QIDP to serve Rock Island and Mercer Counties. A bachelor's degree in a human services related field and one-year cumulative experience providing direct service to individuals with developmental disabilities is required. The experience can be met through employment, volunteer work, or family care. The position offers excellent benefits, a schedule of 8:00 a.m. to 4:30 p.m., Monday through Friday, with flexibility to work partially from home.
General Description Under the direct supervision of the Program Manager of Prairieland Service Coordination, Inc., the Case Manager assesses individual needs and ensures the provision of necessary services through communication, service coordination linkage with provider agencies, service monitoring, and individual advocacy.
Job Responsibilities
Prairieland Service Coordination Inc. provides services in twenty-three counties and a Case Manager may be requested to work in a county outside their usual geographic area due to staff illness, resignation, or as otherwise needed.
Complete the PAS process following the PAS manual procedures.
Advocate on behalf of the individual and/or family to assist them in obtaining services to which they are eligible and have chosen, ensuring direct involvement and participation of the individual and their family.
Review the individual’s medical, residential, vocational, educational, legal, and financial status needs and choices through Visiting Notes as required.
Schedule and complete Discovery and Personal Plan processes annually or as required.
Maintain individual files as specified by program and agency guidelines.
Provide problem‑solving consultation with provider agencies following the DHS Conflict Resolution guideline.
Coordinate specialized services for eligible Bogard individuals as needed and submit summary reports to external agencies as appropriate.
Monitor services provided to Bogard class members monthly and advocate for unmet needs.
Enter billings daily into Birdseye to ensure accurate payroll reporting.
Participate in scheduled staff meetings and mandated training sessions as recommended by the Executive Director and/or Program Manager.
Alert the Program Manager of PSCI to problematic issues, trends, or service gaps.
Perform other related work as required or requested by the Executive Director and/or Program Manager.

Job Qualifications
A bachelor’s degree in human services and QIDP eligibility as established by the Department of Human Services.
Ability to organize, plan, and follow a schedule on a daily, weekly, monthly, and annual basis.
Working knowledge of the developmental disabilities service delivery system, including recipient eligibility requirements and procedures.
Ability to work and interact effectively with professionals, paraprofessionals, social service organizations, and clients including their families.
Ability to work well in teams and with a high degree of self‑discipline and self‑motivation at a home‑office site without direct supervision.
Flexibility to work hours outside the normal workweek.
Willingness to follow directions, accept constructive criticism, and accept responsibility for one’s own actions.
Adherence to the Employee Code of Ethics Guideline through professional diplomatic oral and written communication.
Reliable transportation to and from work, with a valid Illinois Driver’s License, personal auto insurance, and a good motor vehicle record.
Physical Expectations
