About the Role: Join HomeWell Care Services CA305 as a Care Manager RN, where you'll play a vital role in enhancing the quality of life for our clients. We are looking for a compassionate and skilled RN to lead our care team and ensure exceptional service delivery in Upland, CA.
Responsibilities:
Assess client health needs and develop personalized care plans.
Supervise and support caregivers in delivering high-quality care.
Conduct regular client evaluations and adjust care plans as necessary.
Collaborate with healthcare professionals to coordinate services.
Ensure compliance with state regulations and company policies.
Provide education and support to clients and their families.
Maintain accurate documentation of client progress and care activities.
Participate in ongoing training and professional development opportunities.
Requirements:
Current RN license in California.
Minimum of 2 years of nursing experience, preferably in home health or hospice.
Strong leadership and communication skills.
Ability to develop and implement effective care plans.
Compassionate demeanor with a focus on patient-centered care.
Proficient in electronic health record (EHR) systems.
Valid driver’s license and reliable transportation.
CPR and First Aid certification required.
