Registered Nurse – Field Assessor – LTS – Remote
At Brighton/Suncrest Hospice our goal is to change the expectation of hospice care by providing exceptional care and service to our patients. Our strategy involves allocating resources to increase staff‑to‑patient ratios, thereby increasing clinical visits while lowering clinician caseloads. The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside (private homes, skilled nursing facilities, and assisted living facilities). This position is a contributing member of the interdisciplinary team, providing expertise regarding psych‑social assessments, interventions, and knowledge of community resources. The RN will work in either a Case Management, Admissions, or Resource role.

Responsibilities
Provide care and services to patients at various locations.
Conduct psych‑social assessments and interventions, and identify community resources.
Work daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
Deliver 1‑1 care to patients while receiving support from the patient‑care team.
Work independently with phone, email, written communication, meetings, and hospice regulations.
Manage visits, documentation, and meetings; prioritize and triage multiple concerns.
Learn and use the electronic medical records system.
Drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area.
Qualifications
Current RN licensure in good standing in the state(s) of practice.
Strong team‑collaboration skills with interdisciplinary staff.
Independent work capability.
Excellent time‑management skills.
