Provide care and services for a group of patients, visiting patients where they reside, including private homes, skilled nursing facilities and assisted living facilities. Act as a contributing member of the interdisciplinary team, offering expertise regarding psychosocial assessments, interventions and community resources. Work independently managing visits, documentation, and communications while collaborating daily with nurses, social workers, chaplains, volunteers and CNA leadership. Prioritize and triage multiple concerns, responding to urgent needs and scheduling visits. Learn and use the electronic medical record system. Drive within the service area with reliable transportation, a valid driver’s license and auto insurance. Qualifications

Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice. Strong teamwork and collaboration skills. Excellent time‑management and organizational abilities. Strong communication skills, both written and verbal. Ability to work independently and manage own schedule. Valid driver’s license, reliable transportation and auto insurance.
