Job Summary This role is responsible for the health and wellbeing of memory care residents, providing personal care assistance and all activities of daily living. The Specialist will deliver protective oversight, redirection, behavior therapies, and customer service to residents in the Memory Care Wing at The Fountains.

Essential Duties and Responsibilities

Demonstrate ethical and professional behavior aligned with Hilltop’s values and community standards.

Follow all Hilltop, Fountains, and state/federal rules, regulations, policies, and procedures.

Maintain professional boundaries while upholding dignity, confidentiality, and resident rights.

Provide protective oversight: assist residents, maintain safe environments, redirect disoriented individuals, enforce house rules, report difficulties, and monitor visitors.

Administer social supervision: assist with transportation, communication, and participation in social events.

Ensure sanitation and safety of resident areas, removing hazards, maintaining supplies, and redirecting residents from unsafe situations.

Perform personal care in accordance with individual care plans and document care provided.

Monitor ambulation and assist residents to and from meals and activities.

Execute housekeeping duties, including mopping, dusting, trash removal, linen changes, stocking supplies, vacuuming, disinfecting, cleaning bathrooms, bedrooms, living rooms, wheelchairs, and organizing apartments.

Respond to radio, pager, and pendant requests for assistance.

Assist residents with memory‑care activities and social supervision.

Order meals and serve residents; clean and sanitize the dining area.

Apply redirection and de‑escalation techniques for behavioral incidents.

Education & Experience High School Diploma or General Education Degree (GED). One year of direct care experience with the Alzheimer’s population preferred. Experience providing personal care and assistance with ADLs required. Minimum 18 years of age.

Colorado Identification Card

Food Handler’s Card (required within six months of employment)

Hilltop Dementia Certification (required within six months of employment)

CPR Certificate/First Aid Certificate (preferred)

Qualified Medication Administration Personnel (QMAP) Certification (preferred)
