Home Health Aide – Home Hospital, North Region role at Mass General Brigham. This full-time position provides hospital-level care in patients\' homes within the North Region. What You’ll Do

Assist patients with daily personal care needs (bathing, dressing, toileting, ambulation, and other activities) to promote independence, safety, and comfort. Follow individualized care plans, document support accurately, communicate with families and clinical teammates, and report concerns promptly. Travel between patient homes across the North Region; assignments are sent the night before to plan routes and patient-specific needs. Provide care for short 4-hour visits or full 12-hour shifts and contribute to healing at home. Why This Role Could Be a Great Fit

Who You’ll Care For

Adult patients receiving hospital-level care at home. Our patient population includes individuals with acute, complex medical conditions such as heart failure, COPD exacerbations, pneumonia, cellulitis, and dehydration. These patients require close monitoring, mobility support, daily personal care, and compassionate presence as they stabilize and recover at home. You will support patients across the North Region, including Beverly, Salem, Peabody, Lynn, Melrose, Medford, Somerville, Lexington, and surrounding communities. Job Summary & Responsibilities

In this role, you will assist with daily personal care needs, follow care plans, document support, and communicate with families and clinical teammates. You will travel between patient homes; assignments are planned with route and patient needs in mind. You may provide a short 4-hour visit or a full 12-hour shift as part of helping patients heal at home. Qualifications

High school diploma or equivalent required Home Health Aide Certification required A valid MA driver’s license and reliable transportation are required for home visits. Must be able to commute to multiple sites/home visits throughout the day. Additional details: 1 year of home health aide experience preferred; 1–2 years clinical, hospital, or managed care experience preferred; comfortable using basic technology and documentation tools.
