Overview
RWJBarnabas Health is looking to add a qualified Mental Health Clinician in Somerville, NJ. This position provides clinical assessment, crisis intervention, evaluations for hospitalization, and crisis service coordination for children, adolescents and their families residing in Somerset County, within the context of the Children's Crisis Intervention Services (CCIS) system.
Responsibilities / Job Functions
Provide clinical assessment and crisis intervention services as part of CCIS.
Coordinate crisis service planning and dispositions for children, adolescents and families.
Collaborate with the Screening psychiatrist, Unit supervision, and other health care team members to communicate clinical information and treatment plans.
Conduct thorough ongoing assessments in accordance with departmental guidelines in a timely manner.
Deliver patient care in a timely and professional manner and maintain professional behavior per hospital policy.
Describe the screening process and update patient/family progress; document attempts to obtain insurance pre-certification and collaborate on discharge planning.
Gather relevant collateral information and coordinate with provider agencies to ensure continuity of care across systems as needed.
Document clearly regarding discharge planning, teaching, and follow-up referrals, and ensure thorough completion of off-site screening evaluations as required.
Monitor the effects of interventions and update the care plan per hospital policy; maintain accurate records and reports prior to end of shift.
Prioritize Child Outreach services (schools, homes, residential facilities) and provide crisis-related hotline information/referrals in line with Screening guidelines.
Document determination of the most appropriate services/disposition considering least restrictive alternatives; describe patient’s mental illness and dangerousness as required by Screening Law and DMHS training.
Collaborate with provider agencies to coordinate care across systems as needed.
Qualifications
MA/MS degree in social work, psychology or related field
Two years of prior experience/training working with children, adolescents and families in a mental health setting
Experience/training in crisis intervention and assessment
Must possess and maintain a valid NJ State Driver's License
Certifications and Licenses
BLS Certification from AHA

Communicate clinical information effectively with the Screening psychiatrist and Unit supervision
Communicate with other health care team members to discuss patient's treatment plan and disposition

Provide patient care in a timely and professional manner
Adhere to professional behavior per hospital policies
Describe screening process and updates to patient/family
Document attempts to obtain insurance pre-certification and coordinate discharge planning
Collaborate with patient and family in care planning and discharge
Gather collateral information as needed and ensure documentation reflects discharge planning
Ensure documentation reflects outreach and community coordination based on Screening guidelines
