Key Responsibilities
Conduct medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment.
Perform independently under stress and in emergency situations.
Drive between locations, navigating with GPS or other tools.
Communicate significant findings, problems, and changes in condition or environment to the clinical coordinator, family, facility, and other personnel involved in patient care.
Assess health status by obtaining relevant health and medical history; perform a physical exam based upon the patient’s age and history; identify significant health and medical risk factors.
Order, interpret, and evaluate laboratory tests and other diagnostic studies to assess patient clinical problems and healthcare needs; develop a diagnosis by analyzing collected data.
Perform diagnostic and therapeutic interventions consistent with certification, law, and state medical privileges; prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency.
Develop a treatment plan in collaboration with the patient and significant others, including Medicare, CMS, ancillary services, and treatment; collaborate with nursing, medical, and other healthcare team members to ensure quality patient care.
Refer patients to providers as appropriate for consultation or specialized health resources and treatment; ensure continuity of care by acting as a liaison among the multidisciplinary care team.
Supervise nurse practitioners and physician assistants per the Collaborative Agreement.
Required Qualifications
Doctor of Medicine or Osteopathy required.
Current unrestricted State license to practice medicine.
Prior supervisory/management experience.
Demonstrated interpersonal, supervisory, and management skills with effective communication.
Proven organizational development and project management abilities.
Knowledge of regulations and reimbursement in home health care, with proven leadership abilities.
Ability to sit, stand, bend, lift, and move intermittently; lift at least 50 lbs.
Current and valid driver’s license; reliable transportation and valid automobile insurance coverage.
Desired Qualifications
Board certification in family practice, internal medicine, geriatric, or palliative care preferred.
Preferred 2 years of experience in healthcare with a combination of acute care, geriatric, and/or long‑term care practice.
Experience with geriatric patients preferred.
Location
Home Office: 4760 Fashion Square Blvd, Saginaw, MI 48604
Main Service Area: Saginaw and surrounding region; patient services in facilities and private homes.
Hours
Office hours: 8:00 AM – 5:00 PM, Monday through Friday.
Pay Range $150,000 – $200,000 USD
