Overview

The Sanctuary of St. Cloud is an affordable, friendly senior living community. We are located near Highway 10 and County 23 in St. Cloud, MN. Our community hosts 101 Assisted Living and 36 Memory Care apartments. We are managed by Ebenezer, which has provided service to seniors across Minnesota for over 107 years. We are seeking a RN to join our busy team in supporting senior residents with nursing care. The team includes resident assistants, a Director of Health Services, a regional nurse, nurse/LPNs, culinary, activities, and more. This nursing position is responsible for ensuring the safety and health needs of residents are met. Full-time, 80 hours in two weeks, evening shifts with eligibility for evening shift differential. Every other weekend and holiday rotation. Generous benefits including RN student loan repayment worth up to $2,000 a year / $10,000 max over 5 years. Nurse Responsibilities

Maintains current assessment and documentation of resident health status. Performs initial and ongoing assessments per Minnesota Assisted Living regulations and Nurse Practice Act; assessments completed every 90 days per regulatory requirements. Collaborates with residents and/or responsible parties through education and timely communication to ensure appropriate service and placement. Ensures up-to-date service agreements at admission, with changes in services, and annually. Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST. Maintains clinical charts and progress notes in a computerized documentation system. Communicates changes in resident conditions to medical staff and other relevant individuals and follows up on health needs. Coordinates resident health services and facilitates coordination with internal and external providers. Ensures physician orders are signed and implemented per Minnesota Assisted Living regulations. Works with residents and/or representatives to assure delivery of appropriate services and funding. Manages and oversees medication and treatment administration; ensures medications and supplies are ordered in a timely manner. Ensures medication and treatment practitioner orders are renewed at least annually and as needed for changes in medication/treatment. Maintains medication administration records and ensures accuracy of documentation and signatures. Monitors work and performance of licensed and unlicensed staff; coordinates care with staff and addresses care issues. Plans and assigns/delegates work to staff within established guidelines; interviews, trains, and orients new employees as directed. Supports staff to complete assignments and assists the Director of Health Services in monitoring staff performance. Schedules and completes supervisory visits; complies with Federal, State, and Local laws. Assists DHS in scheduling activities that support efficient staff utilization; performs other duties as delegated by the Director of Health Services. Supports DHS to act as RN on call during scheduled times; answers calls and gathers information related to residents’ health and safety; takes appropriate action; works with identified internal and external support providers; may require phone directives or travel to the community; reports to DHS and/or Executive Director if further follow-up is needed. Required Qualifications

Associate Degree in Nursing MN Registered Nurse (RN) License Basic Life Support (American Heart Association or Red Cross) Preferred Qualifications
