Join us as a Registered Nurse / RN, Hospice After Hours On Call at AccentCare. Location: Madison Heights, MI (office location) with coverage across Madison Heights, Farmington Hills, Detroit, Macomb, Port Huron, Southgate, and surrounding areas. Position

Full‑time, no remote/virtual. Salary: $78,000 – $86,000 per year. Schedule: 5 shifts of 16 hours per pay period, 4:30 pm‑8:30 am, alternate weekend rotation. Requirements

Current RN license in applicable state; associate degree preferred. CPR certification required (Georgia direct patient care) recommended. Experience in hospice, home health, medical‑surgical, geriatric, or pediatric nursing. Willingness to participate in on‑call rotation; reliable transportation, valid driver’s license, and state‑required liability auto insurance. Strong communication, organizational skills, Microsoft Office, EMR proficiency; ability to collaborate with interdisciplinary team. Physical requirement: lifting patients over 200 lbs with assistance, transporting supplies up to 20 lbs over 500 feet, frequent standing, walking, sitting, stooping, reaching. Responsibilities

Conduct comprehensive nursing assessments; collaborate with patients, families, physicians, and IDG to develop and evaluate individualized plans of care. Maintain accurate documentation; ensure care plans reflect current patient status; communicate effectively with team. Provide education and emotional support to patients and caregivers; participate in on‑call rotations, admissions, and care transitions as directed. If serving as a Care Manager, coordinate all aspects of patient care including scheduling, medication management, plan of care oversight, IDG collaboration, and supervision of LPN, LVN, and hospice aides; participate in IDG meetings and family conferences. If serving in an On‑Call capacity, respond to triage calls, perform visits and assessments, coordinate care with the Administrator On Call; attend IDG meetings and family conferences; participate in training, committees, and other duties as assigned within licensure scope.
