JOB TITLE:
Case Manager
EMPLOYMENT TYPE:

PROGRAM/LOCATION:
Eastern Region Service Center, Willimantic, CT – Community based position requiring travel to Willimantic, Norwich and Putnam utilizing a company vehicle.

ABOUT THE PROGRAM: The Eastern Region Service Center (ERSC) is a regional case management project administered by Advanced Behavioral Health to assist clients who are medically indigent or eligible for or currently receiving Medicaid LIA. CHR case managers work with the staff of five other agencies in eastern Connecticut to provide assessments and other case management services to clients with substance abuse and/or mental health issues. Case managers engage, encourage, and assist clients in obtaining treatment and housing as well as employment, education, and medical services.
ABOUT THE POSITION:
Utilizes appropriate therapeutic models to aid clients in understanding and managing their symptoms in a variety of environments.
Conducts program-specific assessments and screens as required by program.
Provides case management, collaborates closely with the client’s natural supports and treatment teams, connects to prosocial activities, and provides crisis intervention and de‑escalation as needed.
Demonstrates competency in age-related or specialty issues and developmental needs for each population served, and demonstrates program-specific target population competencies.
Taught and supported the learning of a broad range of skills, including emotion regulation, age-appropriate activities, and competencies for living in a home-like setting. Planned and participated in activities to support and increase skills and competencies.
Documents all services and contacts with clients, collateral contacts, and appointments in compliance with agency standards.
Transports clients as required by program.
Participates in staff meetings, supervision, training and other meetings as assigned.
Additional duties as required.
QUALIFICATIONS:
Education:
Bachelor’s degree in human services preferred, or equivalent education plus work experience required.
Experience:
At least two (2) years’ experience in a mental health setting preferred.
Licensure/Certification/Registration:
Valid driver’s license.
COMPENSATION:
$18.86/hr minimum, and up commensurate upon experience. Actual rates are determined at the time of offer and are based on relevant experience, internal equity, and adherence to our internal equity policy. Many positions offer program/location differentials and/or sign-on incentives (part-time positions are pro-rated).
