Duties & Responsibilities

BHH specialists provide care coordination, health promotion, patient and family support, referral to community support services.

Collaborate with BHH Primary nurse care and other team members to improve medical and behavioral health care coordination and outcomes for enrollees.

Work to reduce health care disparities whenever possible.

Demonstrate competency in age‑related or specialty issues and developmental needs for each population served.

Assist in obtaining access to medical and wellness services including tobacco cessation intervention services, benefits, housing, treatment, legal services, medical services, education, and employment.

Assist with money management and other activities of daily living.

Assist patients with accessing necessary health care including transportation; may provide client transportation as needed.

Provide a high level of collaboration with medical providers to ensure medical follow‑up including PCP & specialist visits, labs, tests, and procedures to manage chronic health conditions.

Participate in hospital discharge process to promote highly coordinated care transitions for behavioral health and/or medical admissions.

Provide wellness and health‑promoting lifestyle interventions such as nutritional counseling, smoking prevention and cessation, obesity reduction, increasing physical activity.

Provide crisis intervention and support as clinically indicated.

Work as liaison between agency and state and private hospitals, other community‑based services, family members and significant others.

Maintain documentation in compliance with agency and program standards.

Participate in staff meetings, supervision, training, and other meetings as assigned.

Perform additional duties as required.

Qualifications Education:

BA in Human Services required.

Experience:

Two years experience in mental health setting; experience working with primary care providers preferred.

Bilingual candidates strong‑preferred.

Licensure/Certification/Registration:

Valid driver’s license; CPR/First Aid Certification within 6 months of hire.

Compensation $18.86/hour min., and up, commensurate upon experience.

Actual rates are determined at the time of offer and are based on relevant experience, internal equity policy; program/location differentials and sign‑on incentives may apply. Part‑time positions are prorated.
