The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus‑based direct services and supports at 13 centers located throughout the state.

Core Responsibilities The Senior Direct Care Registered Nurse performs highly complex (senior level) nursing work, providing assessment, care and integrated treatment of individuals residing at the SSLC. Responsibilities include, but are not limited to:

Performing complex nursing tasks and procedures, administering medications, immunizations and treatments in accordance with HHSC, SSLC and state regulations.

Interpreting diagnostic tests, collecting specimens, communicating findings and recommendations.

Making rounds, coordinating, and organizing nursing services and activities; monitoring and reporting the health of individuals.

Providing consultation, training, instruction, and technical assistance to others; developing in‑service training and health education strategies.

Leading, overseeing, coaching and monitoring tasks performed by LVNs and RNs, acting as a preceptor as assigned.

Performing timely and accurate documentation; working independently with all levels of staff and departments.

Identifying and implementing various courses of action for routine and non‑routine situations in conjunction with other nurses and health care providers.

Analyzing data, evaluating nursing intervention outcomes and providing input to the interdisciplinary team.

Responding to emergency situations in accordance with protocols.

Providing treatment to HHS employees injured in the course and scope of employment as needed.

Performing other duties as assigned, working under the limited supervision of the Nurse Manager or other designated supervisor.

Essential Job Functions (EJFs) Attends work on a regular basis, may be required to work a specific shift schedule, rotating schedule, extended shift, and/or overtime per agency leave policy. Performs other duties as assigned. Performs highly complex and technical nursing tasks and procedures such as tracheostomy care, gastrostomy/jejunostomy care, wound care, and breathing treatments. Administers medications, immunizations, and treatments; monitors patients for unusual symptoms and reactions. Collaborates with RN Case Manager on new admissions, providing detailed initial assessment, collecting data, and presenting key findings to providers and the interdisciplinary team.

Interprets and evaluates diagnostic tests, collects specimens, and communicates findings to nurses, PCPs, and staff. Coordinates and organizes nursing services and activities for individuals on assigned home/unit, ensuring tasks are completed effectively. Monitors individuals’ weight, diet, feedings, and vital signs, and initiates acute care plans as needed. Makes rounds, and may work in other locations as required. Provides consultation, training, and technical assistance to direct support professionals about care needs and monitors post‑procedure signs. Develops in‑service training and health education strategies; advises individuals on meeting health care needs when warranted.

Assists the Nurse Manager by providing leadership and oversight within assigned home/unit/department, coaching and monitoring LVNs, and acting as a preceptor for newly hired RNs. Supports day‑to‑day supervision activities, follows up on controlled drug counts, medication counts, emergency checklist logs, temperature logs, and glucose controls. Assists LVNs and RNs with protocol implementation to improve health and safety, assists with follow‑up assessments, acute care plans, injury reports, restraints, and medication variances. Documents and reports treatments and observations accurately in the EMR, overseeing documentation of others when training.

Works independently, identifies solutions for routine and complex situations, supports infection control and quality assurance initiatives, recommends process improvements, and participates in facility QA/QI initiatives. Analyzes data, evaluates outcomes, and inputs to the interdisciplinary team. Attends and provides information to meetings related to assigned individuals, ensuring nursing issues are addressed. Responds to emergency situations, follows SSLC infection control procedures, provides educational and safety training to staff. Provides treatment to HHS employees injured in the scope of employment. Participates in disaster response or continuity of operations activations as required.

Knowledge, Skills, and Abilities

State and federal laws and ICF‑IID regulations related to State Supported Living Center.

Evidence‑based practices for individuals with developmental disabilities.

Emergency procedures and equipment (CPR, etc.) and ability to assess and care for residents or staff in emergency.

Advanced knowledge of nursing techniques, standards, procedures; health care laws; medical diagnoses; and approved treatment patterns.

Advanced knowledge of nursing care standards and healthcare guidelines.

Skill In

Using computers and electronic health record systems.

Working with individuals with complex physical, nutritional, behavioral, and communication needs.

Excellent verbal and written communication; presenting ideas clearly and concisely.

Maintain current CPR certification and demonstrate safe use of restraint and Ukeru techniques (if trained).

Registrations, Licensure Requirements, or Certifications Licensed to practice as a registered nurse in the State of Texas (or a state that recognizes reciprocity through the Nurse Licensure Compact).

Initial Screening Criteria

Associate degree in nursing (ADN) required.

At least two (2) years of experience as a practicing registered nurse in a health agency, hospital, or health‑care facility required.

Experience in assessment and documentation of health status required.

Specialized training or experience working with individuals with complex medical and behavioral health issues preferred.

Experience working with individuals with intellectual and/or developmental disabilities (IDD) preferred.
