Discharge Planning Initial Evaluation Sending referrals for post acute resources/ placement after hospital discharge DC Planning Reassessment Arranging discharge transportation Minimum Qualifications
LMSW or LCSW; must be licensed in VA BLS; ACLS 2 years recent Case Management Experience in Acute hospital setting Proficient with Meditech and CarePort Care Management for Documentation, referral management Experience with arranging home health services, DME, Home Infusion, Hospice, SNF, Long term care Knowledge of AIDET Preferred Qualifications
ACMA Or CCM Length of Assignment
13 weeks Shift / Hours
Weekdays 5 days per week or 4- 10 hr shifts Thu-Sun or Fri-Mon (8:00AM-6:30PM) Systems
Meditech EMR; Careport Care Management; ECapture Start Date
