Field: This field‑based role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. Candidates not within a reasonable commuting distance from the posting location(s) will not be considered, unless an accommodation is granted as required by law.

The LTSS Service Coordinator is responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs.

Responsibilities

Perform face‑to‑face program assessments, using tools and pre‑defined questions, and apply motivational interviewing for evaluations, coordination, and management of an individual's waiver (LTSS/IDD) and BH/PH needs.

Use identification tools to identify members with potential clinical health‑care needs and coordinate those member's cases with the clinical healthcare management and interdisciplinary team.

Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, and disabilities to ensure cost‑effective utilization of long‑term services and supports.

Document short‑ and long‑term service and support goals in collaboration with the member’s care team.

Identify members that would benefit from an alternative level of service or other waiver programs.

Act as mentor, subject‑matter expert or preceptor for new staff and assist in formal training of associates; may participate in process improvement initiatives.

Submit utilization/authorization requests with documentation supporting the care plan.

Report critical incidents to appropriate internal and external parties (e.g., state and county agencies).

Assist in appeals, fair hearings, grievances, and state audits.

Travel to worksite and other locations as necessary.

Minimum Requirements

BA/BS degree and a minimum of 2 years experience working with a social work agency, or equivalent.

Specific education, years, and type of experience may be required per state law and contract requirements.

Preferred Skills, Capabilities And Experiences

Degree in a health‑care related field.
