LTSS Service Coordinator Elevance Health is hiring a LTSS Service Coordinator to conduct service coordination functions for a defined caseload of individuals in specialized programs.

Field This field‑based role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training.

Responsibilities The LTSS Service Coordinator is responsible for: Performing face‑to‑face program assessments using predefined tools and techniques for eligibility and needs identification. Managing non‑clinical and clinical needs of members with chronic illnesses, co‑morbidities, and/or disabilities, ensuring efficient utilization of long‑term services and supports. Coordinating with interdisciplinary teams, including Medical Directors, to develop integrated care plans. Documenting short and long‑term service and support goals in collaboration with the member’s care team. Identifying members who may benefit from alternative levels of service or other waiver programs. Submitting utilization/authorization requests to utilization management with supporting documentation. Reporting critical incidents to appropriate internal and external agencies. Assisting with appeals, grievances, and state audits. Traveling to work sites and other locations as necessary.

Impact How you will make an impact: Using motivational interviewing techniques to support member engagement. Serving as a single point of contact for member cases and coordinating with clinical teams. Mentoring new staff and contributing to process improvement initiatives.

Minimum Requirements Requires a BA/BS degree and a minimum of 2 years of experience working with a social work agency, or an equivalent education/experience combination. Specific education, years, and type of experience may be required based on state law and contract requirements.

Preferred Skills, Capabilities and Experiences BA/BS degree in a healthcare related field preferred.
