LTSS Service Coordinator (Case Manager)

Hiring near Southwestern Wisconsin in counties: Pepin, Buffalo, Trempealeau, Jackson, Clark, La Crosse, Monroe, Juneau, Vernon, Sauk, Richland, Crawford, Grant, Iowa, Lafayette, Green.
This field‑based role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. Candidates must be within a reasonable commuting distance from the posting location(s) unless accommodation is granted as required by law.
Family Care is a Wisconsin long‑term care program for older adults and adults with disabilities. The LTSS Service Coordinator is responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs, including:
How You Will Make an Impact
Perform face‑to‑face program assessments using various tools and motivational interviewing techniques to coordinate and manage individuals' waivers, behavioral health, or physical health needs.
Identify members with potential clinical health care needs and coordinate cases with the interdisciplinary team to provide comprehensive care coordination support.
Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, or disabilities to ensure cost‑effective utilization of long‑term services and supports.
Document short and long‑term service and support goals in collaboration with the member’s care team.
Serve as mentor, subject‑matter expert, or preceptor for new staff and assist in formal training and process improvement initiatives.
Submit utilization/authorization requests with supporting documentation aligned with individual care plans.
Report critical incidents to appropriate internal and external parties such as state and county agencies.
Participate in appeals, grievances, audits, and state audits.
Minimum Requirements
BA/BS degree and a minimum of 2 years of experience working with a social work agency, or equivalent combination of education and experience.
Preferred Skills, Capabilities & Experiences
BA/BS degree in a health‑care related field.
Assisted living or social work experience.
Waiver experience.
Comfortable using technology.
Willingness to travel to work sites and other locations as necessary.
