Job Summary Baptist Health is seeking a Social Worker to join our team. This is a Full Time Days opportunity.
This Social Worker will be a part of an interdisciplinary care team with the goal to enhance well being by helping meet the needs impacting a person's life through psychosocial support and connection to appropriate resources. There is a focus on alleviating barriers and lessening the burden of medical conditions.
Responsibilities
Assessing psychosocial needs and provide services that enhance patient functioning, emotional well-being and continued access to services and resources in the community.
Providing care coordination.
Providing psychoeducation.
Providing referrals to community services.
Education Required Bachelor's Degree
Preferred Master's Degree
Experience
1-2 years Social Work Experience
Case Management Experience
License and Certification None
